Making an Appointment
CONSULTATION
A referral is required to be sent to our practice before a consultation can be booked. Please note all consultations require a current referral.
Our doctors will review the referral and discuss with our reception staff an appropriate time frame for your appointment to be booked according to the patients’ severity and needs.
Once our reception staff have been advised they will contact you to make the appointment in the required time frame.
You can obtain a referral through the following:
- General Practitioner (GP)
- Surgeon
- other Medical Practitioners
Or you may be referred by a doctor at one of the following hospitals:
- Royal Perth
- Fremantle/Fiona Stanley
- Princess Margaret
- Sir Charles Gardiner Hospital
MINOR/OUTPATIENT PROCEDURE
A Financial Consent form will need to be completed before any procedure can take place. During this process all financial information will be discussed and an appropriate time will be booked for your procedure by one of our friendly staff members.
